La Femme Pelvic Health & Rehab is not currently participating with any insurances. We are an out-of-network provider; meaning, you must pay in full at the time of service. You will be provided with a detailed receipt that you can submit to your insurance company to be reimbursed directly. Unfortunately, if you have Medicare, we are unable to see you, as Medicare does not allow PTs who choose not to participate, to provide and receive payment for, Medicare-covered services. ​

In New York you have direct access to physical therapy, meaning you can receive PT treatment without seeing a physician first, for 10 visits or 30 days, whichever comes first. After that point, if you still need PT services, you will need a prescription from your doctor. Please note: if choosing to submit your receipt to insurance, some insurances may not reimburse your expenses without a referral.

If you must cancel, please provide 24-hour notice. If you cancel less than 24-hours in advance, you will be responsible for a cancellation fee of 50% of the cost of the visit. Please note that this payment is not billable to insurance.
